USING OUR SERVICES:

Equipment Services – Designed to help you source the best mobility equipment for your unique needs and preferences. To access this service, please submit an Equipment Request Form through our website or contact our Case Manager using the details below. Once we receive your submission, our Case Manager will be in touch to offer comprehensive support. This includes helping you identify suitable products, arranging home demonstrations, and connecting you with a local partnered professional who can process and fulfill your order. Additionally, if needed, our Case Manager can assist you in raising funds through our Grant Assistance Scheme (GAS).

To access this service (GAS), please contact our Case Manager using the information below. If you’re a suitable candidate, they will ask you to submit a short supplementary Grant Assistance Form through a direct link they will provide via email. Please note that this form is not publicly accessible on our website to prevent unsuitable applications. Once we receive your submission, our Case Manager will ask you to pay for your preferred payment package.

Due to high demand, you may be placed on a waiting list at this point. However, if you are currently receiving end-of-life or palliative care, your application will be fast-tracked and prioritised.

When our Case Manager is available, they will be happy to assist you in raising funds for the equipment you have identified, trialled, and discussed with a local partnered professional after submitting your Equipment Request Form. We will work with you to develop a tailored fundraising strategy based on your details. This may include identifying suitable grants, assisting you with application writing, and helping you create and grow a crowdfunding page. The amount of grant funding you may receive depends on meeting specific eligibility criteria set by our partner charities. Eligible AYL users typically receive between £2,000 and £3,000 in grant funding by applying to multiple charities. Please note that this process can take three to six months, as each charity has different application deadlines. For users with a fundraising goal between £2,000, we can only offer a combined fundraising approach. For users with an annual household income exceeding £35,000, we can only offer a crowdfunding approach.

By completing the Grant Assistance Form and paying for our support, you agree to and understand the following information:

– You can only access this service from AYL when purchasing from one of our partnered professionals.

– You will need to be open to discussing financial information with AYL to apply for funding.

– Your chosen payment package fee is not refundable and must be paid upfront. You can also upgrade from one tier to another if required at any point by paying the upgrade fee, which is the difference between the chosen tiers.

– AYL cannot guarantee successful sourcing of financial support.

– We will only ask you to complete application forms for charitable grants where you have a strong chance of success.

– We do not determine the eligibility criteria for charitable grants available to AYL users. The decision on who receives funding rests solely with the individual charities.

– Many of these charities also require an independent supporting letter from a Physiotherapist or Occupational Therapist for AYL to apply for funding on your behalf. If you do not have a supporting letter, AYL can arrange for a private supporting letter from a partnered professional at an additional cost.

– All our advice, application templates, and guide booklets are based and modified around cases where applicants have previously succeeded in acquiring funding grants via AYL.

– If your fundraising goal exceeds £2,000, we can only offer a combined fundraising approach.

– If your annual household income exceeds £35,000, we can only offer a crowdfunding approach.

– If you receive UK benefits, there are restrictions on withdrawing funds raised through charity grants or crowdfunding directly into your personal bank account if the total amount exceeds your benefits bank account threshold (usually £6,000). However, Access Your Life (AYL) can hold these funds on your behalf and will implement a three-way agreement between AYL, yourself, and the relevant partnered professional to ensure the security of your money.

We may use other collection methods to recover fees for failed payments or past-due accounts.

Most of the fees listed in the ‘Public Services’ section are for our support services, as AYL primarily refers our users to partnered professionals for their purchasing needs. However, we occasionally offer a small selection of disability-related products for sale during events and collaborations. Additionally, we can sell mobility equipment directly to users in areas of the UK with limited resources, currently only in the North East. For this service, we charge a minimal upfront fee (see below) based on the amount of money you need to raise.
